Two approaches to The IORT programs

In thefirst approach, patients are transported from the OR to the radiation therapy department and the IORT treatment is delivered on one of the department's linear accelerators that normally treats outpatients with external beam therapy.Thus IORT is blended in with the other treatments.

In the second approach, IORT treatments are delivered on a dedicated machine.Dedicated linear accelerators are those accelerators that are used exclusively for the treatment of patients through the intraoperative technique.The word dedicated is used here to mean that the linear accelerator, conventional [X rays(x ray machines for sale) plus electrons or electron-only] or mobile, is used exclusively for IORT.It is recognized here that no more electron-only linear accelerators of the conventional, non-mobile type, will be built in the future. 
Dedicated linear accelerators may be located in the OR itself or in a room in the radiotherapy department.If the former, the room is basically an OR that also contains a linear accelerator.
If the latter, it is a radiotherapy room that is equipped as an operating room.The former is preferred since it is part of the total OR complex and the patient does not have to be moved outside the room.In the latter case, the patient undergoes surgery far from the hospital’s main OR and issues of maintaining sterility of the operating area have to be addressed as well as the problem of what to do if a surgical emergency arises for which the satellite OR is not equipped to cope.There are currently very few dedicated units in the OR in the United States; there are slightly more located in the therapy department. 
For the non-dedicated linear accelerator, the need to transport the patient for each case from the OR to the radiotherapy area is a disincentive to the IORT program.For this reason, programs using nondedicated linear accelerators have seen a decline in numbers (see Fig.4).It is highly unlikely that any more dedicated units of the conventional or electron-only type will be installed in the United States in the future for the reasons outlined above.An example of a dedicated electron-only linear accelerator located in the OR is shown in Figure1.The room measures ~6.1 * 8.0m and the unit is mounted into the wall at one end, ensuring an adequate space for surgery.Experience has shown that centers with dedicated OR suites or mobile systems perform more IORT procedures(automatic x ray film processor) than centers that use the patient transport technique.

Virginia Lifts Restrictions on NOMAD Handheld X-ray

The State of Virginia recently announced that it will drop dosimeter requirements in favor of unrestricted use of the Aribex NOMAD? family of(portable x-ray machine)handheld X-ray systems,effective January 1,2014. 
After careful investigation,the Virginia Department of Health,Office of Radiological Health has determined that,beginning in 2014,personnel dosimetry will no longer be required for those who operate the NOMAD family of handheld dental X-ray systems.The department's determination follows an eight-year period during which they established a safe-use baseline of the product. 
"When the FDA first cleared the NOMAD in 2005,the regulations governing X-ray systems did not allow handheld operation of an X-ray device," said Kaufman."Instead,the rules required that the system be operated from behind a barrier,or at a distance of six feet or more from the X-ray head.Recognizing that technology would outpace restrictions,rule-makers allowed for variances to the limitations when alternative methods achieved the same intended result."
Some states,such as Virginia,California and New Jersey,initially took a cautionary approach in allowing for a variance from these rules by requiring operator personnel to monitor radiation exposure using dosimetry.Such monitoring protocols further confirmed that NOMAD operator dose was as low as reasonably achievable.This year,each of these states have confirmed what dozens of states,studies,and evaluations have found: when used as instructed,the NOMAD is safe and effective. In making this move,Virginia becomes the 35th state with no mandatory personnel dosimetry requirement for operators of the NOMAD. 
"As they should be,radiological devices are subject to significant regulation and concern for safety," said Kaufman."Safety is our number one priority,and we're proud that more than 30 independent studies and evaluations have validated our position by attesting to the safety and efficacy of our products(handheld ultrasound)." 
While fifteen states currently require operator dosimetry in conjunction with handheld use,many are actively reviewing the data to assess whether to allow for similar policy changes. 
"It is no small undertaking to develop a revolutionary technology," said Kaufman."Aribex is gratified each time independent data confirms our efforts to design and produce the safest systems possible."

Computed tomography is a medical imaging method employing tomography created by computer processing. The word “tomography “is derived from the Greek tomos(slice ) and graphein.

In older CT scanners, the X-rays source would move in a circular fashion to acquire a single “slice”. Once the slice had been completed, the scanner table would move to position the patient for the next slice; meanwhile the X-rays source/detectors would reverse direction to avoid tangling their cables. In helical CT the X-rays source are attached to a freely rotating gantry. During a scan, the table moves the patient smoothly through the scanner. The mane of helical CT derives from the helical path traced out by the X-ray beam. It was the development of slip ring technology that made helical CT practical. In the mid 1980’s,an innovation called the power slip ring was developed so that the X-ray cable could be abandoned. The slip ring allows electric power to be transferred from a stationary power source onto the continuously rotating gantry. CT scanners with slop rings can rotate continuously and do not have to slow down to start and stop.


The major advantage of helical scanning compared to the traditional CT, is speed; a large volume can be covered in 20~60 seconds. The scanning time is dramatically shortened.
